Assessment Planning Consultation
OPHI provides assistance to state and local public health agencies on community health assessments (CHA), state health assessments (SHA), community health needs assessments (CHNA) and community health improvement planning (CHIP) and tribal CHAs/CHIPs. OPHI provides consultation, helping partners understand CHA/CHIP requirements, explore models, and develop plans. As a neutral and trusted agent, OPHI provides facilitation, engaging partners, establishing common ground, and leading processes for identifying priorities and planning for change. OPHI enhances the capacity of partner organizations in data collection, analysis, and interpretation and provides documentation of communities’ CHA/CHIP processes through reports and other dissemination materials.
